  • Please arrive 5-10 minutes before the start of a class or training

  • Sign in and/or check-in at the front desk or with your instructor

  • Inform your instructor or personal trainer of any injuries

  • Holm Studio is not responsible for the loss or damage to any personal belongings left in vehicle or while on the premises

CANCELLATION POLICY: ALL SALES FINAL. No credits will be issued for any used or unused classes. No refund will be issued for partial months. No refund will be due or given for change in schedule, times, location or instructor changes.

MEMBERSHIP FREEZE: In the event you need to freeze your account due to extended travels, injury or a medical condition, you must give Holm Studio 14 days notice (sudden medical or sudden injury exception). You may choose the amount of time to freeze account up to two (2) months without incurring additional fees. After that, automatic billing for any auto pay accounts will recur unless fourteen (14) days notice is given.

Holm Studio has the right to alter the hours of operation, the location of the studio, and the right to amend the future cost of, add, modify and/or eliminate any service, class or contract including, but not limited to, Class Memberships at Holm Studio's sole discretion.

We may change these policies at any time without notice, in our sole discretion. As a member or guest of Holm Studio Personal Training, you are responsible and expected to review and comply with these policies at all times.

COVID POLICIES (LAST UPDATED 1/1/2022)

CLASS & PERSONAL TRAINING PROTOCOLS:

•Please limit outside belongings (you may bring own yoga mat and towel....but must stay with you entire time and will be sprayed before leaving)

• Bring own water bottle. 

• We have limited space for purses/jacket so leave in car and/or only have with you that you must. We will provide small baskets that will be cleaned before and after classes for you to put your belongings in. 

• ALL equipment and mats will be sprayed and disinfected before each class and after each class. 

• Please leave immediately after class so we can clean and get ready for the next class.

• DO NOT COME IN IF YOU OR ANYONE IN YOUR HOUSEHOLD IS SICK IN ANY WAY!

• DO NOT COME IN IF YOU OR ANYONE IN YOUR HOUSEHOLD HAS BEEN EXPOSED TO COVID 19!

COVID 19 WAIVER INFORMATION (last updated 6/1/2020)

Holm Studio Personal Training, Inc. cannot remove all risk or otherwise guarantee or promise that you will not sustain any injuries or damages from viruses, communicable diseases, or other health hazards. In consideration for receiving permission to BE ON PREMISES at HOLM STUDIO PERSONAL TRAINING, inc, HSPT (hereinafter the “Activity or Activities”), you must agree to the following before attending any of our classes or trainings (the following agreements and waiver will be provided):

1. I understand the hazards of the novel coronavirus (“COVID-19”) and am familiar with the Centers for Disease Control and Prevention (“CDC”) guidelines regarding COVID-19. I acknowledge and understand that that the circumstances regarding COVID-19 are changing from day to day and that, accordingly, the CDC guidelines are regularly modified and updated and I accept full responsibility for familiarizing myself with the most recent updates.

2. Notwithstanding the risks associated with COVID-19, which I readily acknowledge, I hereby willingly choose to participate in Activities.

3. I acknowledge and fully assume the risk of illness or death related to COVID-19 arising from my being on the premises and participating in the Activities and hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE (on behalf of myself and any minor children from whom I have the capacity contract) HSPT, it’s owners, officers, directors, agents, employees and assigns (the “RELEASEES”) from any liability related to COVID-19 which might occur as a result my being on the premises and participating in the Activities.

4. I shall indemnify, defend and hold harmless the RELEASEES from and against any and all claims, demands, suits, judgments, losses or expenses of any nature whatsoever (including, without limitation, attorneys’ fees, costs and disbursements, whether of in-house or outside counsel and whether or not an action is brought, on appeal or otherwise), arising from or out of, or relating to, directly or indirectly, the infection of COVID-19 or any other illness or injury.

5. It is my express intent that this Waiver and Hold Harmless Agreement shall bind any assigns and representatives, and shall be deemed as a RELEASE, WAIVER, DISCHARGE, AND COVENANT NOT TO SUE the above-named RELEASEES. This Agreement and the provisions contained herein shall be construed, interpreted and controlled according to the laws of the State of Illinois. I HEREBY KNOWINGLY AND VOLUNTARILY WAIVE ANY RIGHT TO A JURY TRIAL OF ANY DISPUTE ARISING IN CONNECTION WITH THIS AGREEMENT. I ACKNOWLEDGE THAT THIS WAIVER WAS EXPRESSLY NEGOTIATED AND IS A MATERIAL INDUCEMENT THE PERMISSION GRANTED BY RELEASEES TO BE ON PREMISES AND PARTICIPATE IN THE ACTIVITIES.

LIMITED CLASSES/WEEK MEMBERSHIPS: These memberships entitle you to a limited amount of classes per month. Memberships are non-transferable and non-refundable. One person only per membership

CLASS CANCELLATIONS: Our online and Holm Studio app schedule are maintained daily and are a reliable source of information for class cancellations or substitutions. We believe in the safety of our clients and staff. If we feel weather conditions may put you or the staff at risk, we will cancel class. We will do our best to provide advanced notice, but please check the schedule on our app prior to coming if you feel weather may be an issue.

CLASS PARTICIPATION: You are not guaranteed a spot in any Holm Studio class as a result of any Agreement. The only way to guarantee a spot in any given class is by reserving online or through our app.

  • We recommend each student bring their own yoga mat to any class. We do, however, keep a limited number of yoga mats and mat cleaner available for use.
